You have rights and responsibilities when you take part in the Healthyroads coaching program.

YOUR RIGHTS

Information

You have the right to:

  • Information about Healthyroads and the coaching program.
  • Program materials that are easy to read, understand, and use.
  • Answers to your questions about the program or your account.
  • Get health care facts that are supported by science.

Respect and Non-discrimination

You have the right to:

  • Respectful, fair, and kind care from Healthyroads coaches and staff.

Health Care Choices

You have the right to:

  • Talk to your doctor about the coaching program.
  • Make choices about your health care.
  • Have a family member or friend take part in your coaching call.
  • Have these rights apply to your legal representative for health care issues.

Personal Health Information

You have the right to:

  • Have your personal health information kept private.
  • Ask Healthyroads to share your health record with your doctor.

Access to Services

You have the right to:

  • Start and end the coaching program when you want to.
  • Know who the coaching staff is and how they are trained.
  • Ask for a different coach.
  • Have Healthyroads work with your case manager or health care advocate, if needed.

Questions and Complaints

You have the right to:

Call Healthyroads to:
  • Ask a question.
  • Make a complaint.
  • File an appeal or grievance.

Minors

If you are the parent/guardian of a minor, you have the right to:

  • Review any information given by the minor. The minor has the right to be told that any information he or she gives can be shared with the parent/guardian.
  • Decide who will take part in the coaching call (minor and/or parent/guardian).
  • End a coaching call at any time.

YOUR RESPONSIBILITIES

You have the responsibility to:
  • Give true health information to your coach while you are in the program.
  • Tell your coach about your wants and needs.
  • Take part in setting reachable healthy goals.
  • Actively work on meeting your healthy goals.
  • Ask for a different coach if you feel your coach is not working out.
